Six years of operation. A clean audit. Zero Commission complaints. On paper, performing. In practice, a 4/20 governance score with five must-fix items before the next audit, a Director Penalty Notice risk active on ATO obligations, superannuation arrears, a $1.1M unreconciled accrued expenses balance, and a confirmed data breach that had gone undetected for 27 days. AOBC engaged in April 2026. Workstreams in active delivery across financial remediation, IT governance, policy rebuild, and clinical standards. The audit was passing. The foundations were not.

AOBC issued an action roadmap across three phases — beginning the week of engagement. What was built in three years is genuinely rare. Our role was to make sure nothing stopped it.

The care was good. The governance was not keeping pace. After the new Aged Care Act commenced on 1 July 2025, this provider's governing body could not demonstrate it was meeting its statutory obligations — only that it had been meeting. Incident management was reactive. A falls pattern had not been escalated. The Statement of Rights was in the policies but not in practice. AOBC assessed the full governance position, rebuilt the board's evidence trail, formalised the quality improvement system, and lodged a continuous improvement plan with the Commission. The board left the engagement able to answer the question they couldn't answer when they arrived.
